California dairies going broke due to feed, milk prices

HANFORD, Calif. ? In nearly six decades of running a dairy in central California, Mary Cameron made a name for herself in a male-dominated industry: She led several dairy organizations and was honored as Outstanding Dairy Producer of the Year.

But the 82-year-old Cameron ? who still drives a tractor and supervises her Hanford dairy ? is on the brink of losing her life?s work. She can no longer pay the bills. Her bank has classified her loan as distressed. And she can?t afford enough feed for her 900 milking cows and 1000 heifers.

"I have been in this business for 57 years and I have never been in financial trouble like I am right now," said Cameron, who runs the Atsma-Cameron Dairy with her two sons. "I?m on the verge of bankruptcy. It?s horrible and inexcusable."

Cameron is not alone. Across California, the nation?s largest dairy state, dozens of dairy operators large and small have filed for bankruptcy in recent months and many teeter on the edge of insolvency. Others have sold their herds or sent them to slaughter and given up on the business.

Experts say California dairymen face a double whammy: exorbitant feed costs and lower milk prices. The Midwest drought has led to corn and soybean costs increasing by more than 50 percent this summer, stressing dairymen from Wisconsin and Minnesota to Missouri. But in California, milk prices have also lagged behind those in the rest of the nation, exacerbating the crisis.

And while milk revenues in California have soared to over $7.5 billion in 2011, making milk the top agricultural commodity, higher revenues mean little, famers say, because it costs so much more to produce the milk.

"I don?t think there?s a milk producer in the state who is profitable right now," said Michael Marsh, CEO of Western United Dairymen.

Since 2008, California has lost nearly 300 dairies, with 1,668 remaining as of January, according to the California Department of Food and Agriculture. There are no official estimates on how many dairies have shuttered in 2012 ? but interviews with dairymen and experts indicate several hundred dairies could be in danger of going under.

"It?s been like a floodgate," said Riley Walter, a Fresno-based agricultural bankruptcy lawyer who has worked on 58 cases of dairies in financial trouble this past year ? from bankruptcies, to liquidations, to operations taken over by receivers.

"Recently, I had two men over 60 years old who broke down and sobbed in court," Walter said. "You would be surprised how much these men care about their cows."

At the Overland Stock Yard in Hanford, owner Peter Belezzuoli said he sees two to three dairymen selling their entire herd every month, compared to about four per year before the crisis. More cows are being sold for slaughter, he said. And the value of dairy cattle has plummeted by as much as 50 percent in the past five years.

"It?s no different than the housing industry, where people lost all the equity," he said. "People have the same cow, but now it doesn?t have the same value."

Economists say milk and feed prices always fluctuate ? but it?s the margin between the two that counts, and how far apart the thin years are.

Only three years ago, falling milk prices forced many dairymen to go under. The current crisis, dairymen say, came too quickly. Many still have unpaid loans, have exhausted their equity, and can?t get new loans.

For Cameron, who grew up washing barns and feeding cows, costs of production vastly surpass revenues. She?s losing $40,000 every month, she said.

Her parents emigrated from Holland in the 1920?s and started a dairy in California?s Central Valley. After college, Cameron followed in their footsteps: Her office wall is filled with awards and news articles touting her successful dairy career.

Mitsubishi targets growth of tiny property funding business

TOKYO (Reuters) - Japan's top trading house Mitsubishi Corp <8058.T> is expanding its small real estate financing business to benefit from rising property purchases by foreign investors in the country and an expected jump in refinancing of loans, a senior executive said.

Diamond Realty Management Inc, a real estate asset management firm wholly owned by Mitsubishi, aims to raise 10 billion yen ($128 million) from Japanese investors in a second fund as early as March to provide mezzanine financing for deals, Diamond Realty's CEO-designate Takashi Tsuji said in an interview.

Mezzanine loans, subordinated to senior debt provided by banks, are used to stretch the amount of borrowing. They were popular during the pre-2008 crisis property boom in Japan when many assets were bought with highly leveraged financing.

After the property market burst in the financial crisis, many providers of mezzanine loans faded away.

But such financing could be in demand now when the property market in Tokyo has started picking up and foreigners are eyeing buying properties.

Angelo Gordon & Co, the $24 billion New York-based asset manager, bought in June the 13-storey Aoyoma Bell Commons property in Tokyo's posh Aoyama district, while MGPA, an Australian property investment firm, bought eight office buildings in the same month for around 12 billion yen.

"It is true that the Japanese banks are willing to lend money, so senior loans are accessible," said Tsuji, who is currently Diamond Realty's chief operating officer and will become its chief executive in November. The company manages about 550 billion yen in assets.

"However there is still a limit for senior lending when the balance of the loans and property value is considered. Mezzanine loans are instrumental in filling the gap between senior loans and equity."

Diamond Realty will approach Japanese pension funds, regional banks and corporations for the fund-raising, Tsuji said. Its first fund, set up in October last year, also raised a similar amount.

REFINANCING OPPORTUNITIES

Mezzanine loans generate higher returns than senior loans, which is good for investors seeking alternative investments when the stock markets are stagnant and bond yields remain low, Tsuji said.

A large amount of loans maturing would create refinancing needs, Tsuji added.

Moody's Investors Service expected about 500 billion yen of loans, which are repackaged as commercial mortgage backed securities, would mature this year, a reflection of the hangover from the last property boom in 2007 when a record 2 trillion yen of CMBS were issued.

Diamond Realty will also start managing its first private real estate trust next month, which is expected to expand to as large as 250 billion yen in five years, said Tsuji.

The REIT will initially start with three assets in retail, logistics and residential properties, worth 32 billion yen, he said.

Mitsubishi's financial business, which includes real estate financing, posted a net profit of 2.9 billion yen for the three months ended June 30, up nearly six-fold from the same period of the previous year. The whole company's net profit for the latest quarter was 98.1 billion yen. ($1 = 77.6950 Japanese yen)

Our brains are better at hearing new and approaching sounds than detecting when a sound disappears, according to a study published today funded by the Wellcome Trust. The findings could explain why parents often fail to notice the sudden quiet from the playroom that usually accompanies the onset of mischief.

Hearing plays an important role as an early warning system to rapidly direct our attention to new events. Indeed we often rely on sounds to alert us to things that are happening around us before we see them, for example somebody walking into the room while our back is turned to the door. Yet little is known about how our brains make sense of the sounds happening around us and what makes us hear certain events while completely missing others.

Researchers at the UCL Ear Institute wanted to try and understand what makes certain sounds easily detectable while others go unnoticed. They created artificial 'soundscapes' comprised of different on-going sounds and asked listeners to detect the onset or disappearance of different sound-objects within the melee.

Overall, the team found that listeners are remarkably tuned to detecting new sounds around them but are much less able to detect when a sound disappears. In busy sound environments, the participants missed more than half of the changes occurring around them and the changes that were detected involved much longer reaction times. The effects were observed even in relatively simple soundscapes and didn't seem to be affected by volume.

Dr Maria Chait, who led the research at the UCL Ear Institute, said: "On the one hand, we might expect to be more sensitive to the appearance of new events. In terms of survival, it is clearly much more important to detect the arrival of a predator than one that has just disappeared. But this reasoning doesn't apply to other situations. Imagine walking in a forest with your friend behind you and suddenly having the sound of their footsteps disappear. Our results demonstrate that there are a large number of potentially urgent events to which we are fundamentally not sensitive. We refer to this phenomenon as 'disappearance blindness'"

The study also explored how resilient listeners are to scene interruptions. In busy scenes, such as those we often face in the world around us, important scene changes frequently coincide in time with other events. The study showed that even brief interruptions, such as a short 'beep' occurring at the same time as the change, are sufficient to make listeners fail to notice larger scene changes. It is thought that this occurs because the interruption briefly captures our attention and prevents the information about the change from reaching our consciousness.

"Understanding what makes certain events pop out and grab attention while others pass by un-noticed is important not only for understanding how we perceive the world but also has important practical applications. For example, to aid the design of devices intended to help professionals such as air traffic controllers and pilots who operate in environments where the detection of change is critical," added Dr Chait.

Social phobia anxiety disorder. What comes to mind? What are the primary signs and symptoms? How is it assessed and diagnosed by clinical psychologists? Fear of attending social events is another symptom. Having poor self-image and low self-esteem is one of the primary signs and symptoms.

Early childhood events and experiences or inherited biological traits are the primary causes of social phobia anxiety disorder allthough these can be overcome. Failure to learn social skills while growing up is the main shared factor in all the cases of social phobia anxiety disorder. Inherited traits occur where the parent may have had social phobia and becomes a world-class warrior about dangers in the world outside the front door.

Some typical examples include shy children raised in isolation by either overcritical and psychologically abusive parents where there is no interaction with other children leading to the development of social skills or the fear of being teased and constant criticism from others because of difficulties with the language, culture and being socially awkward.

Not every case of social phobia anxiety occurs during childhood however. Sometimes during the student years events occur which trigger later social phobia. Such as situation specific events like public speaking or speaking before groups. Any bad experience or traumatic event can lead to the fear of being in social situations and can become genuinely terrifying. Presentations in front of others where there are high expectations can lead to social phobia if it leads to feelings of failure, being useless and being self-critical.

Cognitive behaviour therapy is the best treatment rather than any medical treatment. Social education classes where social skills are learned to increase confidence leading to self-improvement are the answer. Changing negative thoughts to positive thoughts is the real secret to this form of therapy.

Learning social cues and how to respond to them as much as learning psychological cues by cognitive restructuring in self-help groups is the best form of treatment. By challenging the memory of events stored in the amygdala, negative events can be eliminated. Learning to treat others as you wish to be treated so as to establish relationships is the answer to overcoming social phobia anxiety.

UN rights body extends Syria war crimes mission

GENEVA (AP) ? The U.N.'s top human rights body on Friday extended by six months the mission of its independent expert panel probing alleged war crimes in Syria's 18-month conflict.

The body also named renowned former U.N. war crimes prosecutor Carla Del Ponte to the panel, in a sign of stepped-up efforts to gather evidence against members of Syrian President Bashar Assad's regime.

The panel has blamed Syria's government forces for the majority of serious abuses since the uprising began in March 2011. The collected evidence could one day be used in a war crimes tribunal hearing ? although none is so far planned at the International Criminal Court in The Hague, Netherlands.

Along with Del Ponte, the U.N. Human Rights Council also named Vitit Muntarbhorn, a Thai professor who has investigated human rights in North Korea.

Last week, the panel submitted a confidential second list of suspected war crimes perpetrators to the U.N. human rights office. The mandate of the panel, led by Brazilian professor Paulo Sergio Pinheiro, was due to expire at the end of the month.

Members of the 47-nation Human Rights Council voted 41 to three in favor of a resolution put forward by Arab states. Three countries abstained. Russia, China and Cuba opposed the resolution.

The resolution, which extends the mission until the council's next session in March, was drawn up by a number of Arab, Western and developing countries.

Syria's ambassador, Faysal Khabbaz Hamoui, denounced the resolution and lashed out at those who backed it ? in particular Arab states who are funding Syrian opposition fighters.

"They have decided to draft a resolution that is very tolerant to al-Qaida," he said, insisting that Syria will "never become an extremist Islamic emirate."

The U.S. ambassador, Eileen Chamberlain Donahoe, said that the investigative panel "has been doing its job" ? notably to turn up information that could one day be used to prosecute those behind Syria's bloodshed.

The panel's members "will help ensure that this will not be a case where impunity prevails, but rather that those responsible for these crimes against the Syrian people will face justice and accountability," she said.

Ahead of the Bell: Research in Motion

NEW YORK (AP) ? Shares of Research in Motion climbed in premarket trading on Wednesday, as an analyst said that the Blackberry maker's second-quarter financial results may not be as bad as Wall Street is expecting.

Shaw Wu of Sterne, Agee & Leach says Wall Street is anticipating that Research in Motion will report a large decline in quarterly revenue and a big operating loss. The company will report its results on Thursday.

"With low expectations, we believe there is a fair chance that the company may meet or even slightly beat," Wu wrote in a client note.

Analysts polled by FactSet expect a loss of 46 cents per share on revenue of $2.43 billion.

Still, Wu says the second-quarter performance may not matter, as investors may focus more on the Canadian company's cash balance and its job-cutting plans.

In June, Research in Motion said that it would eliminate 5,000 jobs as part of a previously announced initiative to cut $1 billion in annual costs this year. The job cuts represent approximately 30 percent of RIM's workforce of about 16,500.

Wu says RIM had a cash balance of $2.25 billion in the first quarter. While it is anticipated RIM will face an operating loss in the second quarter, the analyst says that investors will be looking to see if the company was able to hold onto or even grow its cash balance by collecting on accounts receivables and working capital changes. This was something RIM was able to do in the first quarter, Wu explained.

The analyst reaffirmed a "Neutral" rating.

William Blair's Anil Doradla is concerned that RIM may not meet his quarterly revenue estimate of $2.3 billion as the company deals with intense competition and a weak product offering to consumers.

"Blackberry smartphones remain an afterthought in the consumer market," Doradla wrote.

The analyst says he doesn't believe there will be any major improvement in RIM's share price until the Blackberry 10 rolls out, and even then it remains to be seen if the new device will be embraced by consumers.

Doradla maintained a "Market Perform" rating.

Research in Motion Ltd.'s stock added 18 cents, or 2.7 percent, to $6.78 before the market open. The shares have lost more than half their value since the beginning of the year.

Before we begin, a disclaimer: We're not doctors. We spoke to several for this story, but don't think you'll be able to walk away from this post able to diagnose your own symptoms. Our goal is to help you find reliable sources to turn to when you have minor aches and injuries, and give you an idea of what's up if you can't get an appointment right away. Keep in mind though, if diagnosis were easy, doctors wouldn't have to train so long to learn how to do it. If you have a serious condition or don't know what's going on, always consult a doctor as soon as possible.

How to Get Reliable Medical Information on the Internet Without Turning Into a Hypochondriac We all know what it's like: you have an ache you're not familiar with, or a bruise suddenly appears and you have no idea where it came from. You fire up Google and type in the symptom, as specific as you can make it, and you're instantly overwhelmed by the dozens of possibilities, keywords, illnesses, and other scary terms. Unfortunately, no site can diagnose an illness based on symptoms that you type into a search box. Sure, you probably know if your stomachache just happened to set in a few hours after visiting the oyster bar, but keep in mind that a heart attack can have symptoms very similar to a bout of bad indigestion, and it can be difficult even for trained and experienced professionals to tell the difference without equipment or specific tests.

Every doctor I spoke to for this story warned against using Google for diagnoses, and noted that the world of medical information on the internet will just make you crazy with wonder if you search blindly. No one wants to sit in the dark at their computer at 2am wondering if the ache in your leg is a pulled muscle or a pulmonary embolism when one site says to rub some athletic cream on it and go to sleep while another says you need to go to the ER immediately. So what do you do instead?

Search Smart, and Use Interactive Doctor/Patient Services That Really Help

How to Get Reliable Medical Information on the Internet Without Turning Into a Hypochondriac Scatter-shot symptom searches are almost always a bad idea, but there are some services and sites that are better than others. In most cases though, a cool head is much more valuable than a page full of scary search results. When you're struggling with symptoms, here are some important things to remember:

  • How confident are you that this is minor/major? There's no foolproof way to tell a minor issue from a major one, but things like pain or discomfort level can help. Try to stay calm, especially if the symptoms are unfamiliar, and think things through. Is it getting better or worse? Have you experienced this before, and how does this time compare? If you think the issue is major or don't know what you're dealing with, seek professional help. If the issue is minor, remember that as you research, so you don't get carried away.
  • Search with Occam's Razor in mind. Several of the doctors I spoke to mentioned that some patients wind up panicking because they dug up search results that aligned with their symptoms, but were so unlikely that they're almost never correct. This is one of the biggest issues with searching by symptom on the web: someone with a bad cold may come away thinking they've come down with dengue flu just because the symptoms may be similar. Remember, common conditions are the most likely ones (eg, a sprained ankle not a broken one, or a headache not an aneurysm.) Still, leave diagnosis to the pros: see a doctor so they can look you over and run tests if necessary?just be open to the fact that whatever you found online may not be the issue at hand.
  • How to Get Reliable Medical Information on the Internet Without Turning Into a HypochondriacLearn to tell good medical sources from questionable ones. This point comes up frequently in the Quora thread we mentioned earlier, but it's critical. Healthfinder.gov, The Mayo Clinic, Medpedia; they're all great sites, but those sites are good because their articles and other content written by, vetted, and reviewed by actual medical professionals before they're published. When you're reading about the symptoms you're experiencing, make sure the information was written or at least reviewed by a doctor, and you're not reading the comments of an otherwise informative article or some user forum where everyone's a self-proclaimed doctor.


    Also, make sure the doctors you're reading are experienced in the areas you're researching?a nutritionist can tell you how to eat well, but they can't tell you if your twisted ankle needs heat or cool to get better. Stick to established, trusted sites with authoritative voices, and when in doubt, print out what you're reading and ask your own doctor for a final say. Remember, nothing you read on the internet is a substitute for real, in-person analysis by a physician. It can be informative and useful as a way to talk to your doctor, but never the final word.

  • Use a service run by doctors that connects you with doctors. A number of new services and apps have appeared recently to combat this issue. One of the leading services is HealthTap, a service we've mentioned before that even has mobile apps to connect you with doctors on the go. One of the doctors I spoke to uses HealthTap himself to connect with people looking for help and offer guidance based on the information they provide?and of course to suggest they see a professional when necessary. The app has a database of questions asked by patients and answered by doctors who have signed up to help, and if you find a doctor you like, you can follow them to see more questions they answer, or communicate with them directly. The app even helps you find a doctor in your area and make an appointment.


    HealthTap really shines when you have symptoms you want to know about though. Before you ask Dr. Google, the apps "virtual private consultations" allow you to pose your question to a live, on-call doctor, ready to offer their best opinion based on the symptoms, photos, video, or other information you provide. They still cost money ($10 per conversation), but it's still cheaper than many co-pays, and gets a real professional opinion in a short period of time. If you're experiencing something unfamiliar and don't want to go to the ER, or between health insurance providers, it's a useful way to talk to a doctor about what you're going through. HealthTap isn't the only service that puts real doctors on your computer screen or on your smartphone, but it's the best we've seen.

  • How to Get Reliable Medical Information on the Internet Without Turning Into a HypochondriacCall an Employee Assistance Program (EAP) instead. Instead of taking to the internet with your symptoms, check your health insurance plan (if you have insurance, that is) and see if an EAP is available to you. Many EAPs have 24-hour hotlines you can call with both physical and mental health issues in case you're on the fence about whether you should go back to bed or head to the ER. Usually it's an on-call nurse that'll answer, or an operator who'll hand you off to a doctor or nurse, but it's better than symptom searching. Like HealthTap and similar services, an EAP puts you in touch with a live person who can help. Several years ago I fell for the marketing lingo in a commercial for an allergy pill I was interested in. I went to the doctor and explained that I had seasonal allergies, and would love something to help alleviate the symptoms. They wrote me a prescription, but it wasn't for the drug I was thinking about. I felt like an idiot?even more so looking back on it with what I know now?but let my embarrassment be a lesson: you don't want to be an "internet patient," so caught up in marketing language and far-fetched diagnoses that you can't tell a sniffle from internal bleeding, or worse, won't listen to the doctor when they explain it.

After the Diagnosis: Do Your Homework and Ask Questions

Similarly, if the doctor prescribes medication, you can read all about it and its side effects as well.

The web's wealth of medical information is really only useful to individuals with health conditions after you've talked to a doctor, but any doctor will tell you that an educated and engaged patient is a good one. Don't hesitate to do your homework, but go in to conversations with your doctor with an open mind. If you're looking for medical information before seeing a doctor, make sure you take what you find with a grain of salt, and if you're really concerned, make an appointment, visit the ER, or use a service that connects you with a doctor right away.

Germany lists EADS/BAE worries while UK starts inquiry

BERLIN/LONDON (Reuters) - Germany has deep reservations about proposals to merge Airbus parent EADS and Britain's BAE Systems , an official document showed, as the British parliament began an inquiry into the planned European defense and aerospace giant.

Underlining the complex domestic and international politics surrounding the proposed deal, British Prime Minister David Cameron has also contacted the leaders of France and Germany, seeking to safeguard UK interests should it go ahead.

The $45 billion merger would be the biggest shake up of Europe's defense and aerospace industry in a decade and touches on the sensitive issues of safeguarding jobs and military relations with Washington.

Berlin's worries, revealed in an economy ministry document obtained by Reuters on Monday, include doubts about whether the combined group would be safe from takeovers and could guarantee jobs when even Germany is being hit by the euro zone crisis.

The document marks the latest sign that the planned merger between EADS, which is strong in civilian aviation, and the British defense group could run into trouble due to disagreement between Paris and Berlin over the degree of government say in the future company.

EADS, which is controlled by a group of French and German public and industrial interests, has warned it would walk away from the deal with BAE that was publicly proposed earlier this month if the new group were to suffer excessive state meddling.

"A key point of the discussions with EADS is the question of take-over protection," said the document, which is addressed to a German parliamentary committee investigating the plan.

EADS and BAE have said they will offer the governments of France, Germany and Britain a "golden share" in the new company, which sources say is aimed at preventing a hostile takeover.

"France and Germany agree the 'special share' offered by the company does not offer a complete guarantee, as this can possibly be challenged by European law. Possibilities for achieving an effective take-over protection are under discussion but will be difficult to defend legally," the document said.

Golden share arrangements can and have been challenged within the European Union. In 2003 the European Court of Justice ordered the British government to give up its golden share in airport operator BAA, for example.

Three years later Spanish infrastructure firm Ferrovial bought BAA, which owns London Heathrow and several other British airports, for 10.3 billion pounds.

EADS and BAE together employ tens of thousands of workers across Britain and continental Europe. The plan dominated a meeting of German Chancellor Angela Merkel and French President Francois Hollande on Saturday, but the pair failed to reach any common decisions. They are, however, striving for a joint position, the document said.

It said guarantees provided on future location of factories were not reliable, underscoring fears about possible job losses if the plan goes ahead. EADS has about 50,000 employees in Germany across 29 sites.

RATIO UNHAPPINESS

German carmaker Daimler , French media firm Lagardere and the French state all hold substantial direct stakes in EADS.

The document showed Berlin is unhappy with plans to value EADS at 60 percent of the combined company. Sources have also told Reuters that Daimler feels the deal undervalues EADS.

The paper said the 60-40 ratio "does not correctly reflect the actual value which is closer to 70 to 30".

Based on recent share prices, the ratio would have been nearer 50-50 in 2010 but in the last few months, BAE's share of the overall market capitalization has fallen below the 40 percent mark.

Berlin has no direct stake in EADS, but had planned to acquire part of Daimler's holding through state development bank KfW. The acquisition plan has been "significantly complicated and delayed" by the proposed merger, the government paper noted.

EADS CEO Tom Enders will appear before German parliament on Wednesday morning to put forward the case for a combination with BAE Systems and answer politicians' questions, German paper Die Welt said in an advance copy of a story to be published on Tuesday.

EADS and BAE have until October 10 under UK takeover rules to set out detailed plans for the merger, although there is speculation they may seek an extension.

In London, a spokesman for Cameron said the British leader had spoken to Merkel on Friday and Hollande on Monday about the issue. BAE employs 35,000 people in Britain, while EADS has about 15,000 employees in the country.

"Given the nature of the companies' activities the government will clearly have some involvement and we need to ensure that the UK's public interest is properly protected," the spokesman said. "We are talking to the companies to ensure that that is the case."

BAE, which also has 40,000 staff in the United States, has a Special Security Arrangement with the Pentagon that allows it to work as a subcontractor on classified contracts.

BRITISH INQUIRY

Britain's cross-party parliamentary defense committee launched an inquiry on Monday into the possible merger.

The influential committee said it would take evidence in October and November to establish the impact any merger would have on the protection of British sovereign capabilities, defense exports, Britain's relationship with the United States and European allies, and jobs and trade.

The two companies' manufacturing sites in Britain include BAE's shipyards in Scotland and EADS' Airbus wing factory in north Wales.

"BAE are leading talks with the British government but EADS are very much involved - they (the government) are seeking assurances on jobs from the Airbus side as much as the BAE side," said a source close to BAE.

Potentially adding to the complications, the EU and Washington have traded allegations for years in a dispute over state aid to EADS and U.S. group Boeing .

In Geneva, the United States said it had complied with a ruling ordering it to withdraw unfair subsidies to Boeing as tit-for-tat exchanges flared up in the world's largest trade dispute.

The World Trade Organization (WTO) found in March that the U.S. planemaker had received billions of dollars in unfair aid, following an earlier ruling against European support to Airbus.

Washington has targeted government loans to Airbus, which it says helped the European planemaker compete unfairly to replace Boeing as the world's largest commercial jetmaker. The EU says Boeing is unfairly assisted by government research deals and other federal and local measures.

High oxygen levels after surgery linked to increased long-term risk of death

Patients given high concentrations of inhaled oxygen during and after cancer surgery may be at higher long-term risk of death, according to a report in the October issue of Anesthesia & Analgesia, official journal of the International Anesthesia Research Society (IARS).

Although preliminary, the study by Dr Christian S. Meyhoff of Copenhagen University Hospital and colleagues provides new evidence on possible risks -- in addition to previous data questioning the benefits -- of administering 80 percent oxygen to patients undergoing major surgery.

80 Percent Oxygen Linked to Higher Mortality Two Years after Cancer Surgery

The researchers evaluated long-term follow-up data from the PROXI study -- a randomized clinical trial performed to evaluate the effects of high concentrations of inhaled oxygen on infection rates after surgery. About 1,400 patients undergoing abdominal surgery were randomly assigned to receive either 80 percent or 30 percent oxygen in the perioperative period: during surgery and for two hours afterward.

In contrast to previous results, the original PROXI study found no difference in the risk of surgical wound infections. There was also a trend toward an increased short-term risk of death with 80 percent oxygen: 4.4 percent, compared to 2.9 percent in patients receiving standard treatment with 30 percent oxygen.

The increase in mortality was not statistically significant. However, it was enough to prompt a follow-up study to determine whether there was any real increase in the risk of death among patients receiving high concentrations of inhaled oxygen -- not only in the short term, but also in the subsequent months and years.

At one to three years after surgery, there were more deaths in the high-oxygen group. The long-term mortality rate was 23.2 percent for patients assigned to 80 percent oxygen, compared to 18.3 percent in the 30 percent oxygen group.

After adjustment for other factors, the difference was significant only for patients undergoing cancer surgery. In this group, the use of 80 percent oxygen was associated with a 45 percent increase in the risk of death. For patients undergoing other types of abdominal surgery, there was no difference in mortality between groups.

The long-term difference in mortality was surprising, because it appeared unrelated to any immediate harmful effects of 80 percent oxygen. (Breathing high concentrations of oxygen can cause harmful effects on the lungs in some cases.) The authors speculate on some possible mechanisms by which high oxygen concentrations could specifically affect the behavior of cancers, potentially increasing the risk of recurrence.

Dr Meyhoff and coauthors emphasize the preliminary nature of their findings -- the study was not designed to detect a difference in long-term mortality risk between 80 percent and 30 percent oxygen. They write, "Further studies are required before an association between perioperative hyperoxia and long-term mortality is established beyond reasonable doubt."

In the meantime, the findings add concerns over long-term mortality to the original PROXI results showing no reduction in infections among patients assigned to high concentrations of inhaled oxygen. The researchers conclude, "Until a clinical benefit of 80 percent perioperative oxygen is well-documented, we recommend abstaining from administering [oxygen concentrations] above what is needed to maintain sufficient arterial oxygen saturation."

(AP) ? Mitt Romney led a chorus of Republican criticism of the administration's foreign policy on Monday, accusing President Barack Obama of minimizing the recent killing of the U.S. ambassador to Libya as a mere "bump in the road" rather than part of a chain of events that threatens American interests.

White House press secretary Jay Carney called the accusations "desperate and offensive," an attempt by Romney and his allies to gain political advantage in the latter stages of a political campaign that seems to be trending Obama's way.

The president did not comment on the criticism when he and first lady Michele Obama taped an appearance on ABC's "The View" that blended the personal with the political. Asked if a Romney presidency would be a disaster, Obama said the nation can "survive a lot." He added: "The American people don't want to just survive, we want to thrive."

The back and forth on foreign policy occurred as Romney said he was shifting to a more energetic schedule of public campaign events, bidding to reverse recent erosion in battleground state polls. After days spent largely raising campaign cash ? and trying to minimize the fallout from one speech to donors last spring ? he pledged to make the case for "real and positive change."

While national polls make the race exceedingly close, Obama has gained ground on Romney in many recent surveys when potential voters are asked to compare the two rivals in their ability to fix the economy. Sluggish growth and national unemployment of 8.1 percent make the economy by far the dominant issue in the race, and the two men have focused much of their time and advertising budgets on highlighting their differences on taxes, spending and plans for job creation.

The same polls show Obama with a healthy lead over Romney when voters are asked which candidate is better equipped to handle foreign policy, and the president has not shied away from trumpeting his decision to order the secret mission by U.S. forces that killed terrorism mastermind Osama bin Laden in his Pakistani hideout more than a year ago.

At the same time, Romney's advisers say voters are more inclined to question Obama's handling of foreign policy after the attack on the U.S. consulate in Benghazi, Libya, earlier this month resulted in the death of the U.S. ambassador and three other Americans.

Not only Romney, but other Republicans, as well, challenged Obama on foreign policy on Monday.

In a conference call with reporters, Rep. Eric Cantor, R-Va., the House majority leader, said, "Israel continues to find itself on the receiving end of harsh language by the president of the White House. ... There is a somewhat continued pattern of throwing Israel under the bus when Israel stands as our closest ally."

And the National Republican Senatorial Committee issued challenges to Democratic candidates in several races to "share their view" on Obama's remarks in an interview with CBS' "60 Minutes" over the weekend.

In the interview, Obama was responding when asked if recent events in the Middle East gave him pause for supporting governments that came to power following a wave of regime changes known as the Arab Spring.

He said he has long noted that events were going to be rocky, adding that the question itself "presumes that somehow we could have stopped this wave of change."

"I think it was absolutely the right thing for us to align ourselves with democracy, universal rights. ... But I was pretty certain and continue to be pretty certain that there are going to be bumps in the road because ? you know, in a lot of these places ? the one organizing principle has been Islam. ...

"There are strains of extremism, and anti-Americans, and anti-Western sentiments and you know can be tapped into by demagogues," he added.

Romney was eager to talk about the topic, squeezing interviews with three television networks into his schedule and touching on the subject at the beginning of a rally in Pueblo, Colo.

"I can't imagine saying something like the assassination of ambassadors is a bump in the road, when you look at the entire context, the assassination, the Muslim Brotherhood president being elected in Egypt, 20,000 people killed in Syria, Iran close to becoming a nuclear nation, that these are far from being bumps in the road," he told ABC.

"They represent events that are spinning out of the kind of influence we'd like to have. We're at the mercy of events rather than shaping the events in the Middle East."

U.S. officials are investigating the deaths in Libya, which occurred when the consulate was breached.

In his appearance on "The View," the president avoided a direct answer when asked if the attack had been terrorism.

"There's no doubt that the kind of weapons that were used, the ongoing assault, that it wasn't just a mob action. What's clear is that, around the world, there are still a lot of threats out there," he said.

Romney intends to return to the subject of international affairs and discuss foreign aid, trade agreements and international development when he addresses the Clinton Global Initiative in New York on Tuesday, according to a person familiar with the campaign's thinking.

Romney, like Cantor, took a slap Monday at Obama's handling of relations with Israel.

"The president doesn't have time to actually spend time with leaders of these nations, particularly Bibi Netanyahu, I find that very troubling," he said.

In a campaign setting records for television advertising, both campaigns released new commercials during the day. And Obama conceded some of his own had gone too far.

"You know, do we see sometimes us going overboard in our campaign, the mistakes that are made in areas where there is no doubt that somebody could dispute how we are presenting things ? that happens in politics," he said during the "60 Minutes" taping. The remark was not part of the broadcast but was posted to the CBS website.
